By confirming your massage appoint, you hereby agree to the following:

I understand that Briana Randall is providing massage therapy services within her scope of practice as defined by the Association of Bodywork & Massage Professionals.

I hereby consent for my therapist to treat me with massage therapy for noted purposes including such assessments, examinations and techniques, which may be recommended by the therapist. 

I acknowledge that Briana Randall is not a physician and does not diagnose illness, disease, or any other physical or mental disorder. I understand that massage therapy is not a substitute for a medical examination. It is recommended that I see my personal physician for any ailments that I may be experiencing. I acknowledge that no assurance or guarantee has been provided to me as to the results of the treatment. I acknowledge that with any treatment there can be risks and those risks have been explained to me and I assume those risks. 

I understand that Briana Randall must be fully aware of my existing medical conditions. I have completed my medical history form as provided and disclosed to Briana all medical conditions affecting me. It is my responsibility to keep the massage therapist updated on my medical history. The information I have provided is true and complete to the best of my knowledge. 

I have read the above noted consent and I have had the opportunity to question the contents and my therapy. By confirming my appointment, I confirm my consent to treatment and intend this consent to cover the treatment discussed with me and such additional treatment as proposed by Briana Randall from time to time, to deal with my physical condition and for which I have sought treatment. I understand that at any time I may withdraw my consent and treatment will be stopped. 

By ordering any packages or services, you agree to be bound by these terms & conditions:

• By placing an order with Randall Wellness you warrant that you are at least 18 years old or have written consent from a parent or guardian.

• All personal information provided or obtained by the client to Randall Wellness will be handled by Briana Randall directly and will not be shared with any third party.

• Events outside control of Randall Wellness shall be considered force majeure.

• Before starting a massage program with Randall Wellness, please consult your physician to determine if such program is right for your needs. 

• Do not start a massage program through Randall Wellness if your physician or healthcare provider advises against it. 

• The content and other services and information presented by Randall Wellness is in no way intended to diagnose, cure, or treat any medical or other health condition. The Services, including any Content, does not constitute medical advice and is not intended to be and should not be used in place of the advice of your physician. 

• The price applicable is that set at the date on which you place your order.

• Payment fees and plans are recognized and agreed upon before confirming the purchase.

• Cancellation policy is such that sessions are non-refundable, but session time is flexible.

• 5 hour advance notice or extenuating circumstances are required otherwise client will lose a session credit.

• Card information is transmitted over secure SSL encryption and is not stored unless specifically requested by you through the creation of an online account..

• Please note that local charges may occur.

• Randall Wellness reserves the right to amend any information without prior notice.

Please read these terms & conditions carefully before purchasing any services from Randall Wellness. You should understand that by purchasing these services, you agree to be bound by these terms & conditions. No other terms or changes to the terms & conditions shall be binding unless agreed in writing signed by Briana Randall.

PERSONAL INFORMATION

All personal information you provide Randall Wellness with or that I obtain will be handled by me directly. Personal information provided will be used to ensure deliveries to you, to provide offers and information on massage services to you. The information you provide is only available to me and will not be shared with other third parties. You have the right to inspect the information held about you. You always have the right to request I change or delete the information held about you. By accepting these Conditions, you agree to the above.

FORCE MAJEURE

Events outside control of Randall Wellness, which are not reasonably foreseeable, shall be considered force majeure, meaning that Briana Randall is released from contractual obligations to fulfill product and services agreements. 

PAYMENT

All products and related services remain the property of Briana Randall until full payment is made. The price applicable is that set at the date on which you place your order. You must be at least 18 years old or have written consent from a parent or guardian to purchase from Randall Wellness

All transfers conducted through Randall Wellness are handled and transacted through third party dedicated gateways to guarantee your protection. All card information is handled over SSL encryption. Please read the Terms & Conditions for the payment gateway chosen because they are responsible for the transactions made.

If any provision of this Agreement is declared invalid or unenforceable, the remaining provisions of this Agreement will remain in effect. Briana Randall has the right to modify these terms and conditions at any time.
